*  Team Initial Deposit of $350, regardless of situation all players must pay the deposit to secure spot on team
* Concussion Awareness Form
* Medical and Travel Form

*MYSL Player Contract
*Copy of Birth Cerificate (if New Player)
* Player Headshot size 1"x3/4"

Players 1998- 2003
*MYSL Player Contract
*MSPSP Player Contract
*Copy of Birth Certificate (if New Player)
* Player Headshot Photo size 1"x 3/4"

Directors Academy and/or MRL Players
Paperwork will be emailed out once team made
